Passport Name Change

If you are legally changing your name you must update your passport immediately.  If you are assuming or unassuming your name from marriage or separation there is no time limit to when you should change your name.

You will need to complete the entire application form regardless of your reason for name change, and weather you currently hold a passport or not.  You will need to submit the entire application along with 2 identical passport photos and original ID (or photocopies of your ID that has been signed by a guarantor).  A standard passport is $87.
